Cagliostro doesn't have any biology because they are just a vessel housing a soul of a master alchemist.

In Cagliostro's very first Fate Episode in the Gacha tells you that Cagliostro was a well renounced Alchemist that was a man, Katalina was surprised that Cagliostro's didn't look like a man when they first met and so Cagliostro explained on what they did. Cagliostro doesn't have any biology because they are a clay doll.
