Has to do with the cumulative development levels of the districts but I haven't seen any specific numbers.

medium need 6 total level, big need 10 and massive need 14

each city has commerce, agriculture and barracks level, total level of 3 facility need to reach certain level for it to grow example you have Chang An a city with 2 commerce level 3 agriculture level and 1 barracks level, meaning Chang An is level 6 city as 2+3+1=6

depend on the skill, siege skill use apropriate siege weapon ship skill used while in ship and basically anything else is good with everything beside awl

If you haven't seen yet, there are events for every city upgrade. So you can check the list to see the requirements.

Also other events that you may be able to do so worth peaking through them once in awhile.
