First off, satisfaction has nothing directly to do with how many stars a room has. The type of guests you are catering to is more important. A 5 star room might impress a Noble but will do little to satisfy a Distressed. This means you really have to focus on who you wish to cater to, trying to make an inn that will please all 5 factions will never work. Focus on one or two exclusively. You are much better off making your entire inn one uniform decor then trying to have a room for distressed, with rooms for nobles and townsfolk on the side. You cant control where guests will wander, and you can completely undo guest satisfaction if they wander into a room they wont like.
As far as how to generate satisfaction, you just need to look at your guests.

Every guest will enter your inn with a list of things they want to do. In this example, this guy just wants to use the outhouse. If a guest wants a service and you can not provide it, or provide it poorly, they will not gain happiness. If you provide it well and timely they will gain happiness. They will also gain or lose happiness by being in rooms they like. Pay attention to guest feedback to see where you need to adjust or improve. Satisfaction is only tallied when a guest leaves, each happy guest that leaves should earn you a single satisfaction point. In this example, this guy is very happy based on the gold face icon on the top of his page. He is also a regular, as noted by the gold star, regulars are guests who are so satisfied with your inn they continue to come back. Fame points are earned when you earn satisfaction from guests. As you earn farm the number of guests you need to satisfy increases. You can see what is required here

If you are doing all this and observing happy guests leaving your inn without your satisfaction improving, then its safe to say the game has a new bug.
Edited to add more info on how satisfaction relates to fame.
The by far easiest way to get positive reviews is decorating the main hall in a way that most visitors like. "I like the decor of this room" is a +30 mood modifier, "poor service" is a -10, so someone loving the decor will still be satisfied even if the service is poor.
Next thing is pricing. "the prices are low" is a - i guess - +10 mood modifier. So open the menu tab and move the slider for the price so that the face next to it is smiling. This is usually somewhere near 1/3 from the left side.
