2) You can change difficulty, and there is an ironman mode. Pretty sure you can change during a playthrough, although I haven't tried. There is a 'difficulty' option in the pause menu, though.
It's a fun game, and pretty funny too. Super polished.
You get a new 'prisoner' which has new traits
However any upgrades you hvae researched are maintained through deaths. So if you research something that gives you more health, your new prisoner will have more health when they start.
The map is functionally endlesslly horizontal but limited vertically.
The vertical are as you go down on the map, you encounter harder enemies but better loot.
The horizontal is more just staying in the same 'difficulty' of ships and the same category of loot (certain loot only spawns in lower levels of the nebula)
Yeah, when you die, you start over with a new character that has random perks. But you keep all the gear you have crafted, it carries over. Materials, fuel, food, and ammo don't, but you can craft/upgrade your "care package" which gives your new character more starting loot.
