2) I think it increases or decreases the amount of blood spilled by the enemies.
3) Not sure about this as well, but from here: https://steamcommunity.com/app/837610/discussions/0/1760230157495698142/, it seems like it causes enemies to spill more blood, allowing you to go into Massacre mode more easily, while healing more when in Massacre mode as well.
Tested "bleed damage" skills a bit more and I think it was just the Blood Lord's passive that threw off the results. They really do cause bleeding just from hitting an enemy (no overkilling or hitting a weakness), but only for the user, and it doesn't seem to stack with overkilling. Could still be useful to generate blood against bosses if you can't hit a weakness.
Positive Blood says "increase bleed in [character's] attacks" when used, and it seems like it might work as advertised, i.e. attacks from that character cause more bleeding. Negative Blood presumably does the opposite.
