I have no clue where to head next then if it's not the shrine. People seem to talk about the Underground a great deal but I have no idea where it might be...
Hint? Been wandering aimlessly for hours now.
You can easily find and kill two more Guardians without setting foot in the Underworld.
I would wager you haven't found the entrances to the areas where these guardians are found.
-Roots of Yggdrasil
-Immortal Battlefield
-Annfwn
-Icefire Treetop
-Divine Fortress
-Valhalla
-Gate of the dead
-Place of Malice (can't recall the exact name).
-Dark Lord Place
-Takagamahara Shrine
That's about it I think.
I'd like to know in which area I should investigate next as to reduce the scope of possibility here.
I have Earth, Fire, Sea, Heaven and Sun.
Things causing me trouble:
-I don't have the cog.
-I can't get access to the Moon Mantra although I've destroyed the wall leading to it. It's on the other side of a one way door.
-A light machine is trapping some guy which I can't stop.
-I've found the cave of Echidna but I don't have the Scream mantra.
-I need to find a scythe but I don't know how to get to the Eternal prison but apparently that's for later.
Just a hint would be great.

You can find and kill a Guardian in the Dark Lord's Mausoleum. That place has a lot of puzzles you won't need to solve until the end of the game, and aren't necessary for finding and killing its guardian.
I assume you met Amen-Ra in the Dark Lord's Mausoleum, and then he flew off somewhere. Where do you suppose he went? The answer isn't "the Ankh", not unless you saw the big shining light that lets you know an Ankh has appeared. He gave you a hint as to what his plans were before he flew off, and the Ennead had some ideas what his plans were as well. Go check back with them if you forgot.

I did all that and got the light but I'm missing the moon mantra to go down there.

I did, trust me. The only entrance I could imagine would be from the bottom in the Icefire treetop. There's a ladder there but there's no way to access that ladder either! I'll investigate more.
As for the room under the Taka Shrine, I managed it once but I'm pretty sure it was a glitch as I slid through the floor of the room above after getting the chest there.
EDIT: Oops, ok nevermind, I can't believe I missed that.

You're overthinking this one. You need to get into that little area in Icefire with the ladder. How can you get in it? Try every angle.

No glitches needed. The only thing stopping you from getting through that door at the bottom of Takamagahara Shrine is how much you want to get through it.

The cog is a lategame item, dont worry about it.
Have you beaten Ratatoskr in the Immortal Battlefield yet? He also appears somewhere in the Treetop, Im not sure what triggers it but you can unlock another area by fighting him there.
And after beating the invulnerable boss something changes in the room. You probably did not notice it, so if you dont want to look for yourself or cant find it: in the bottom left you can push the block into the wall and get to another room
I've done both. He stole the chest at the Treetop and disappeared after.
Regarding the block, I tried what you did but it didn't seem to work. I'll try again.
Regarding the Mausoleum, it turns out I am missing another mantra. I can get three of the four cats to light up. The fourth one isn't moon, sea or fire, I've tried them all.
