2. I don't think there are any unique idol, they got random name, stats, trait, and looks. There are a lot of random options enough, so you rarely see one that look the same (and you can change their hairstyle, cloth, and accessory if need).
Silver, Gold, and Platinum idol, have higher soft stats cap, but you can still max stat for any ordinary idol if you want, it just take more time.
3. So far, I found only one idol that will only appear with condition to get. A little spoiler here.
You need to bankrupt second time, and some rich guy will pay you money for allowing his daughter to become idol with some privilege for a year. You also get Nepotism achievement by doing this, however her stats is very low (not sure that her stats are random or not).
Unique idols are common in many mods. Unique just means you can't have multiple of the same idol (generally the one(s) added by the mod). I don't know for certain whether a unique idol can be generated during auditions again after the previous one graduates/retires. I would assume not.
Many unique idol mods can be easily modified to make uniques non-unique. You can find subscribed workshop items in (Steam\steamapps\workshop\content\821880). You can randomize stats by removing all stat based lines in their parameters.json. You can randomize rarity by removing the value="X" line. Age, trait, and name can all be randomized by removing the associated lines.
I had that question in the past, and trust me, people would misunderstand it no matter how much specific detail you went into.
