I am having the same issue as well. Girlfriend joins, she saw me at first in one spot when I was not even in that spot. Then she restarted the game and couldn't see me at all. Once she was able to finally see me, I was running in the same place non stop.
She couldn't see the hut I built at all for the longest time, couldn't see the material I had cut down in a pile to use for adding onto the building, and on top of all that a tribal person spawn on her, and was attacking her. She couldn't see them until I started attacking it and died.

There may be many reasons why this might happen. It sounds like desynchronization between players. Have you noticed this issue with other players (if you had a chance)? All players with the issue should try these methods:
- There is a process called „Killer Network Manager“, which is currently pre-installed on some systems, that uses the Killer Network Chip.
After stopping/disabling the service, the issue should be resolved. (This solution seems to fix the issue for most players).
- All players participating in the session should Verify Integrity of game files in the Steam menus. Right click on the game -> Properties-> Local files -> Verify Integrity of game files. After that if everything is ok, restart Steam. Unfortunately sometimes not all files are updated properly so it's good to do it manually to make sure all files are in place.
- If using Antivirus add Green Hell as an exception. Also add Steam to your Firewall. Reinstall the game. It will prevent from blocking files that needs to be accessed by the game.
- Try Port Forwarding. Sometimes ethernet providers might have blocked some ports by default.
- Please note if any player is using mods the game won't run properly and it can be the result of that.
Just in case you can also check:
- Make sure all players meet PC requirements for the game to run properly.
- Make sure all drivers are up to date (WHQL- non beta version).
- Check RAM usage when running a game, there might not be enough free memory for the game to run properly.
