even thought I have gone over the areas numerous times the grappling gun at 27 w 22 s and the cave at 25 w 27/26 s not showing. I have the grappling gun.

Go to Airport then find the diving equipment then head east turn right go down the rope get the oxygen tanks and swim the caves you will then head down the another long rope into cave and out into the swamp area head right to find the omega camp
The grappling Gun Hook is found at the island with little man> Story and survival.

The grappling Hook is found at the over turned jeep in Survival mode
located near (44 W, 16 S)

In Story mode it is replaced with a red petrol can, it is at the Gold Mine use the lift to go down.

That grappling gun indicator to the north east of the airfield never showed up on my map2 too - even with extensive searching on the up- and downplace.

But:
That cave marking the exit from map2 to map3 is shown on both of my ingame maps 2, 3. Maybe approach it from a different side / angle?
And:
According to STEAM the Tribal Village was my 57/60 and 'Cartographer' unlocked with finding the first cave from the tunnel to the south east of the Tribal Village - should have been my 58 (or 59 if both tunnel ends were counted at the same time). There also were some marked places ingame (2x Scuba, some Hooks, Delta-Camp-Fire) that Steam never counted for me - as far as I was/am aware. I am not sure what went on ...

If you already have explored those tunnels I mentioned ... you seem to have missed one (important but hidden) place in the tunnel/caves to the north west of the tribal village wich leads to the pre-marked Delta-Camp. Maybe check that area (again)?
