Click rmb at bone in ur backpack, and select harvest. Or just select harvest, i didn't played this game since a month or something, so i might be wrong:)

Kenji89_PL is correct. You right click a bone, harvest it and it will become a bone needle. Alternatively, harvesting any fish will result in fishbones or stingray spines which can be used to remove the worm.

Once the worm is removed, you will want to bandage it with Molinera to prevent infection/fever.

to avoid worms sleep on hammocks or logbeds ... passing out or sleeping directly on ground is a 80% chance worm .... sleeping on leaf bed is 50% sleeping shelters 40% idk for soem reason shelter sleeping idont run into worms much
