But, I've had some times where the quest disappears for me, then shows up few minutes later.

That's why, if you really don't want to miss anything, the best approach is this:
*When entering a region for the first time, make sure to travel around in order to uncover all of the "?" symbols.
*Avoid killing leaders or "unique-named" NPCs/mercenaries that might be running around as that can complete quest objectives even before starting them. For example, if you kill the leader of the Silver Islands, doing that will make disappear one of the best side quest-lines fom the game.
*If there are main quests in the region, do them last. First focus on the sidequests and once you have completed all of them proceed with the main quests.
As fas as I remember, there are only missable quests in the following regions: Attika, Boetia, Elis and the Silver Islands (Mykonos and Delos).
There are also some "missable" quests in Arkadia, but those are different because you are presented with the option of 2 groups of quests. So, basically you are only supposed to complete one of those, and if you complete both you lock yourself of the "best ending" for the main quests of that region.
Also, when killing mercenaries, a quest pops up saying something like, 'by accident I killed the target of a quest and I can get the rewards now', without having the quest before I killed the guy.
On the other hand, I accepted a quest to kill some hunter ladies, then left this region and probably because of leaving, the quest disappeared from my list.
A little bit irritating is to have my bag full of quest items.
Some are left overs of finished quests. eg I still have the trophy of the legendary wolf even after handing it over to the quest giver.
Some are random quest items I have no l clue what to do with them. eg I picked up some letter or so from a random NPC, announced to be a quest item but no quest started and I arleady cleared the whole region and did all quests.

Or, it was on a timer. And sometimes the quest-giver can be killed. In either case, you're not missing out on anything but random loot, cash and XP and there's a million other random side tasks you can do.

No idea why this would bug you. I kinda like have a collection to look at and reminisce.

This likely means you already got the item. Many of these letters just direct you to a nearby "?" which leads to a cave with a chest of treasure in it. If you found that "?" cave on your own and looted it, then the letter serves no purpose. You can collect it and read it but you already looted but if no quest pops up it likely means you did the quest already.
I accepted this quest from one of the boards. The quest didn't have its own icon on the map. In the end it was also some kind of dynamic quest I guess. One of the always available 'kill x spartans' quests.
I didn't care much before I picked up one quest item without getting a quest. When looking at my inventory I stated to ask myself, how many of those (no-)quest items I already picked up.
I don't know. And I cannot search the internet because I don't know which one are 'forgotten' items for already finished quests, which items are for quests I completely missed, which are for current quests but needed later, which maybe needs additional activities to trigger, which are not guided quests... like the two maps I got from a cult leader. i guess I am supposed to find those places myself, without quest and without map marker?! I noticed those maps just by accident because my bag is already full with useless junk.
Meanwhile I don't care much about those items anymore. There is too much other content to do and very likely not worth the time to do internet research for all of those items in my bag.
No. As i said, I just got another item saying it belongs to a quest but without guidance or quest or anything.
It was not one of those items that trigger a quest. Like i looted a random guy (who was killed by another random NPC), got a letter and a new quest to go to some cave to loot his treasures from a chest. There was an entry for this quest in my list, there was a map marker, there was a notification saying 'new quest...'.
But the other item I picked up didn't have anything of those. And I cannot even say what this item I picked up was because my bag is full of quest items.
Anyways, it seems this is offtopic and Madam Thank Kwan has a completely different problem.
Loosing all quests seems to be happening to some players.
Sometimes progressing in main quest line temporarily disables everything else until you finish this stage in main quest.
Some people said going on/off the ship helped?
Maybe also try fast travelling somewhere?
Some people said it fixed itself after starting the game a couple of times?!
