Assuming you rescued them. Phoibe and Kassandra/Alexio kept living, although MP shook hands with the father and Phoibe standing right next to them.
Further the father told himself, they should visit the doctor first to get appraised/cured.
There's also another part which confirms that they are still alive: When you visit the Pythia in Phokis, if you rescued them, she's gona tell you, "the child and her parents are still alive".
Objectivly the plague could have spread through visitors, goods, other villagers who escaped.
EDIT:
Another option is to skip the mission, ends up with the family being executed off-screen and the plague still spreading.
I saw you feign ignorance in Kausos. You let an unknown sickness govern your decisions. Innocents died thanks to you.
It doesn't matter if you save them or not and that's easy to prove because if you go back later in the game to that location you will find the man and woman's body (but not the children's) If you let them be killed the dead bodies will be there if you go back after going to Athens, If you let them live the bodies will still be there if you go back after going to Athens because they dies from the plague. Also if you completely ignore that quest then the plague spreads to the whole island and you will find the husband and wive's bodies there. Basically no matter what you do they die and you will find the two dead bodies there (This was confirmed by the developers)
Here is a better question ..... With all plagues some people don't get sick but still become carriers of the disease. So the question is "Did Phoibe or Kassassndra/Alexios carry the disease with them to Athens and cause the plague there?"
Here is an interesting interview with the developer that wrote that quest and others in Odyssey as well as several quests in The Witcher 3. At about 25 minutes in he talks about choices and this quest and at 40 minutes he gives some interesting background on Phoibe and why she was included in the game.
