Pyhtia makes it sounds ominous as if you did a wrong thing rather than a right thing.
She can't see the future, the Pythia is a fraud, that's a major reveal in the early game. She learned about those things from the Cult's spies when they prepped her for your inevitable meeting.
No matter how she responds to your actions, she's just feeding you bullpucky at that point. You don't get any truth from her until you speak in private.
You basically have two bad choices (kind of like real life) Either you kill 4 people and save hundreds or you spare 4 people and kill hundreds including the 4 people you spared. While Ubisoft doesn't show the children if you go back later in the game to the same place you'll find the 2 dead adults no matter which choice you make they die
The developer that designed that quest as well as the choice system also developed the choice system and several side quests in Witcher 3 most notably the Triss side quests and the side quest where Geralt gets tied to a bed and left if he tries to romance both Yen and Triss
I mean, logically speaking, if you never do the quest, the priests should still kill the family, thus making Kephallonia safe.
Really, in the event the MC goes along with the priest, this quest is just us showing up, being told it's none of our business, and answering "okay" before leaving.
