How to ignore The Blood Fever Quest?
Is it just by not talking to Phoibe?

Yes, though why ignore it? This quest theme of the right choice not always being immediately evident happens several times in the game, it's a feature.

Because i am going for (If the player ignored The Blood Fever and spared the thieves in Megaris)

Pyhtia makes it sounds like you are an idiot who let sickness govern you choice.

Pythia: Oh, but I do. While in Kausos, you didn't cower in the face of the priest. A child and her family still live thanks to you.
Pyhtia makes it sounds ominous as if you did a wrong thing rather than a right thing.

I've also just wondered what this is about and why she claims we did something wrong if she has the gift and can see the future then she knew perfectly well that if we didn't kill these people the plague would spread across Kefalonia and thousands of innocents would die.

She can't see the future, the Pythia is a fraud, that's a major reveal in the early game. She learned about those things from the Cult's spies when they prepped her for your inevitable meeting.

No matter how she responds to your actions, she's just feeding you bullpucky at that point. You don't get any truth from her until you speak in private.

You do realize if you fight the priests the island ends up devastated by a plague and most of the population dies. Not sure what happens if you skip the quest but I imagine it has the same results, a plague devastates Kephallonia

You basically have two bad choices (kind of like real life) Either you kill 4 people and save hundreds or you spare 4 people and kill hundreds including the 4 people you spared. While Ubisoft doesn't show the children if you go back later in the game to the same place you'll find the 2 dead adults no matter which choice you make they die

The developer that designed that quest as well as the choice system also developed the choice system and several side quests in Witcher 3 most notably the Triss side quests and the side quest where Geralt gets tied to a bed and left if he tries to romance both Yen and Triss

Thousands more did died from the Plague of Athens, even though when the family on Kefalonia did died.

Most are truths by the power of the Pyramid (Isu) used by the cult to influence the Pythia, others could be lies.

If the choice defines the fate of Plague of Athens, it would be impressive that the killing of the family would avert the Plague of Athens, but sadly no it does not establish it. What you say is true that if the Blood Fever Quest is done, the family that is spared the Kefalonia will indeed be devastated by a plague, but as I did not do the quest, I still find that Kefalonia is not devastated by the plague. As I return to the scene of the encounter, I find the parents are missing, the priest, guards and the 2 children are still within the area and that Kefalonia is not devastated by the plague.

I mean, logically speaking, if you never do the quest, the priests should still kill the family, thus making Kephallonia safe.

Really, in the event the MC goes along with the priest, this quest is just us showing up, being told it's none of our business, and answering "okay" before leaving.
