Wolf's Fate failed

So I'm on the quest to return to Elipnor after dealing with the Wolf, but the way I play the game is I explore feckin' everything before I do the main quests.. But I found myself doing the oracle quest and after I interrogated her, she told me some things about Elipnor and the quest I was on "Wolfs Fate" failed.. I looked into it a little and it seems the quest just skipped ahead a bit.. I was still given the rewards I would have otherwise gotten from the quest, but I've obviously missed some dialogue and other things.. This isn't the first time this game has reacted oddly to me exploring the map ahead of when the game expects me to, like early on on Kefalonia, I triggered a phoebe quest before even talking to her about it, apparently I got her friend killed or something.. Is the game going to do this a lot? Should I start treating it like a linear game and finish all quests first then run around exploring after?

Don't worry about it, it was just the way the game is meant. You had to choose between talking to the Oracle or return the Wolf's helmet to Elpenor. You failed the quest because you choose the Oracle and she told you "that" about him, so no point in returning the helmet to him no more because you'll obviously kill him at the moment. Is just a different outcome.
I didn't find any other failed quest screen through the rest of the game, buy maybe there are some instances when, depending on your choices, the same happens again.
