It isn't triggered by attacking, using an ability, dodging, parrying, overpower attacks, charged attacks. It seems entirely random in nature.
It's just a 20% chance. It triggers whenever that is met, regardless of number of hits.
Sure, but whether that's one hit or ten, it's really up to you to see how far you can go for that. Personally I just equip the Greek Heroes set and set some additional Ignore Damage or Elemental Resist engravings and I'm literally unkillable. I may not do much damage but I can tank everything including both soldiers and groups of Mercs.
Having checked it out, I think it is the health shield. Though weirdly it triggers if I'm attacked while using an ability, during which i wouldn't take damage anyway.
