Funnily enough, after seeing this for the first time I then had a maggot and a corrupted shield bearer use their melee attacks on me from miles away for a different save file the day after!
I wish I could tell you what triggers it because I'd really like to know myself!
I'd say if it changes the outcome of the level for you then quit to menu and restart the mission. I was lucky as I had 600 armour on both Blademasters when the latest glitch happened although being rooted to the spot made me a little unhappy...
If it happens again, do try and make note of it on here. Perhaps we might come across a pattern and the Devs could fix it.

it was a monster from the 4rth world (dont want to spoil you if you haven't seen it yet) but not a scavenger (he oneshotted my daughter so i had to redo the whole run.
It seems if a monster spawns too far away from your daughters (for example in the long maps of rescue missions, he can attack on spawn as if it was melee range for some reason)
It's a pretty consistent bug sadly
