What those do is when you hit controler button, it makes game think you hit the keyboard or mouse key that corsponds with that button. So if you have g as throw nade on keyboard. And set it so x on controller acts like g. Whenever you hit x the game thinks your hitting g and do you with throw nade.
They have gotten better but still might be slight delay.
I played BO2 with my controller on Steam and didn't have any problems keeping up with mouse and keyboard players once I got used to not having the aim assist that console versions of COD have.
mr. mayhem maybe so but overall players do better with keyboard and mouse. if you go against not so great of players sure controller will work just fine.
as we said though for cod4 the game itself does not support controller. but 3rd party software is out there and not so bad and it will allow you to use a controller. with speeds these days the reaction time is pretty much not noticeable.
