If that doesn't work, go into Sound <> Recording and if you didn't see a stereo mix there, then enable whatever pops up in the recording tab.
Its always a problem with the sound it seems like :) Glad its working, enjoy CoD4 ;)

a small windows shows a message of " iw3mp.exe has stop running "

https://ccm.net/forum/affich-125400-cod-4-multiplayer-doesn-t-start
was so annoyed after trying to update punkbuster, change punkbuster files to add cod4 trying all sorts with nothing working.
on windows 10 do the same just go to start or press windows icon, search "control panel", go to "hardware and sound", double click "sound", go to "recording" tab, right click on "stereo mix" and click "enable" and voila cod4 mp starts up, magic.
1.Go to Start then Control Panel
2.Double click on the Sound Tool
3.Click on the Recording tab
4.Enable the Stereo Mix
or
plug in a mic / headset also works
