This is proof to me that he was originally supposed to survive, but the devs changed this in the final version
Well, if I'm honest, those 3 objectives that weren't used don't make much sense to them:
- Get radiation equipment. For what? After the fall of the helicopter, why is that going to be obtained, when in the end sooner or later Jackson will die from the radiation left by the bomb when it exploded.
- Locate the commanding officer. This objective does not make sense. They've all died, who are you going to locate? It's logic.
- Find secure overhead cover. Yeah, I'm sure Jackson will find her.
He should survive, that could have been the intention of the developers, but I think you have to be a bit realistic and think better what the fate was for this soldier.

this means that he is confirmed dead, if he wasnt confirmed dead it would say "MIA" instead of KIA
The only realistic way they would have got out is if they hadn't stopped to rescue the pilot, they would have had more time to get out and be outside the blast zone.
