Diao Chan never spawns

I've played MH start like 3 times already and I've never seen Diao Chan spawn, what am I doing wrong?

Playing MOH is what you're doing wrong. Pretty sure that event (and a number of other events) is just plain broken currently.
She still spawns just fine in the 190 start.

Diaochan doesn't spawn if you're not playing as Dong Zhuo. I'm sure this is true even for 190. While you do see the event firing up, the AI doesn't actually get her for some reason.

Dunno if Diaochan joins Dong Zhuo or not if you're playing as someone else, but I know she can still spawn. Possibly enters the random recruit pool.
I've seen a screenshot on Reddit where someone's claimed that after a battle against Yellow Turbans they had the 'Prisoner Recruited' random recruit event which yielded Diaochan.
And I know I've seen Diaochan in Yuan Shao's army once while playing as Sun Jian.

I did play as Dong Zhuo, she still didn't spawn.

So far I've not actually seen her appear in a campaign, although I guess there must be other ways she can appear in besides the event chain with Dong Zhuo if what your saying is true.

Right now it seems that all CA did was replace the generic version of her that is generated by Dong Zhuo's event chain when you're playing as him but didn't make any adjustments to the AI version of this event, which never made her physically spawn. Plus this event never seems to actually trigger in MoH even if Dong Zhuo managed to take control of the Emperor regardless of whether or not he is AI or player controlled.

But I guess she might follow a similar process to other event spawned characters, where if the window for them to spawn passes there is a chance they are simply added to the general recruitment pool or generated through some other random event.
