Report a translation problem
1. You chose to kill/banish the emperor instead of serving as his regent
2. You remove him from a post (minister/administrator) - this happened to me a lot but it seems to fire even if you switch his posts
3. He may also get that trait if you declare yourself emperor as well, but I'm not 100% sure on this. I usually hire him after I already was emperor in the game.
At that point, he'll get the "heartbroken" trait and a unique ancillary "Empty Lunch Pail" - it's supposed to mirror what happened in the novel and real life (as well as the 2010 TV show) where Xun Yu opposed Cao Cao's ambitions to become King of Wei instead of serving the emperor as a Han general. In response to this, Cao Cao sent Xun Yu (who was not eating for days in protest) a box of "fruits," but when Xun Yu opened the box, it was empty. He then took a sword and committed suicide. The box was interpreted as basically Cao Cao saying: "There's nothing left for you to do here."
The historical record did not have the lunch box event as far as I know, just that Xun Yu "conveniently" died at the same time Cao Cao became King of Wei and many historians think that there are some foul plays involved since the frictions between the two were reaching boiling points at that time
Both Wu and MTU add the "empty lunch pail" ancillary I think
