Report a translation problem
Try to find "C:\Users\YOURNAME\AppData\Roaming\The Creative Assembly\ThreeKingdoms\scripts" and open preferences file. There try to put game on borderless windowed mode and see if it helps

Later uninstalled the AV, cleaned the reg with ccleaner, and reinstalled it. Not done it since.

Might be related. Try disabling it and see what happens.
I assume you've tried alt-tab in and out, then seeing if you can click anything.
I also tried it with a different mouse. The most basic mouse I could find and still nothing
Check in the taskbar (default, bottom right of the screen) open up the down arrow, and see what's running in there. End them one by one, with the game open (not steam, or others you need, but a whole lot of crap tends to collect itself there). See if that allows you to click.

to remove them from your startup, and reboot.
Also run the cleaner, and regcleaner.

I just found a mouse button controller which is probably the culprit. Every other person who had this problem fixed it by disabling a mouse related script or hotkey. Unfortunately i started a reinstall now so will have to wait a few hours to see
I have to give you credit, I had no idea I had the thing even on my computer and only found it from the startup menu. So thank you sir
