HOW TO GET "DIAOCHAN'S REVENGE"
I mean my issue now is for Dong Zhuo to even spawn
for some reason he never spawns

I play as Ma Teng
Soon as game starts, i divorce Ma Teng and his wife
then "receive marriage" with Dong Zhuo, pay so and so for Lu Bu to marry Ma Teng's Ex-wife
then Lu Bu joins army
wait a few turns, but Dong Zhuo never deployed

Always his Guo Si, Li Jue and random guy

and another stack consisting of only Zhang Liao.

but no Dong Zhuo.
and no sooner or later he dies for some reason (Flames of War cutscene)

How to get "Diaochan's Revenge" if Dong Zhuo never spawn as an army?

My Ma Teng strategy:

Turn 1: Attack Wudu town, have Ma Teng wounded by the arrow towers. Retreat the rest of your army. Divorce Ma Teng. (Optional: trade with Han Sui for the Jincheng Silk Trader) Always save as much money as you can.

Turn 2: Offer yourself as a vassal to Dong Zhuo, without a guarantee of autonomy + request Lu Bu marries Ma Teng's ex-wife + request Dong Zhuo hands over the Hanzhong Silk Trader + give him a few ancillaries or whatever he needs to say yes. Attack Wudu town, have Ma Teng killed by the arrow towers, capture the town. Have Lu Bu replace Ma Teng as the leader of the retinue, make Lu Bu your heir so that he stays with you, then recall Lu Bu and Pang De.

Turn 3: Redeploy Lu Bu and Pang De at the Hanzhong Silk Trader. SAVE YOUR GAME! Once you do this, recruit as many units as you can afford.

Turn 4: You may wish to wait a turn here for some extra mustering power, otherwise skip to turn 5.

Turn 5: Start walking your army past Chang'an towards Shangyong town. You will continue to replenish your forces as you are a vassal of Dong Zhuo.

Turns 6-9: Keep heading towards Shangyong town. Watch to see what is going on with the army led by Guo Si (or perhaps Li Jue, I forget). Keep moving to intercept when they annex Shangyong town. Note that they start to muster more troops after doing this.

About turn 10: You should be nearly at Shangyong town. Declare war on Dong Zhuo. End turn. He will move his army out of Shangyong town to attack you - play out the battle and be sure to kill the generals, but not wipe out the whole army. The badly wounded remnants will flee back into Shangyong town and Dong Zhuo will spawn in to replace one of the dead generals.

About turn 11: Attack Shanyong town being defended by Dong Zhuo with your army being led by Lu Bu.

You may need to reload your manual save back on turn 4 once or twice to get the exact timing of the movement and composition of your army right.

Worked a charm. I had a little variation there (my green general died instead of Ma Teng) and they were only as far as the horse tamer on turn 15, but I managed to get it. I retreated my whole army and just used my generals to assassinate one of theirs and then took off running. Next turn the Dong popped in and was able to get both achievements.
