Is Disappearing of Gensokyo the first Touhou game in english?

If you mean on Steam, yeah, it's seem.
But in general no, all the Touhou official games have English patch (except 15.5 atm) and most of fanmade games also support english, even if I believe that the said patch are fanmade.

Koumajou Densetsu had an official English translation and that was... 2011 I think.

Probably incorrect to say "most fangames also support english" as there are zillions of touhou fangames. A lot of the better ones have english patches, though.

Maybe that's the first official translation or maybe not, I don't know, but I believe the first Touhou game to get an actual official U.S. release was Genso Rondo, and that was in 2016.

Yes indeed, was mostly refering to best and mostly know, but thanks for that correction.
