controller not working properly
I'm using a ps5 controller to play the game but i decided to take a break from playing for couple of days and now when i hook up my controller, it registers as a Xbox controller instead? does anyone know how to fix this issue cause its starting to get on my nerves

Did you disable Xbox controller support?

1. Go to Steam > Settings > Controller
2 Enable Steam input for Playstation controllers.
3. Disable the other controllers.

You should enable this one: https://imgur.com/a/59w8y2B
If that doesn't work, try reinstalling your controller drivers (see Device Manager in Windows).

I've already tried the options you have suggested and its the same result.

IIRC you probably want to do the opposite and disable Steam Input for SCARLET NEXUS as it natively supports the DualSense controller, do you also have some kind of XINPUT (Xbox Controller emulator) software running e.g DS4Windows or DualSenseX?

Having those running will interfere with the game's native DualSense controller support and will cause it to show Xbox button prompts instead.

i did what you said but the issue is still there
That's weird because I get the proper DualSense prompts with Steam Input disabled using a wired connection. Have you tried updating the DualSense controller firmware by using Sony's official DualSense firmware updater for PC?

i have now check and its said its up to date
