Playing only in the principal (Acer X34 100hz Predator with Gsync) the game runs without problems.
Hope the developers will take action with a pretty quick fix.
So I close the game, do some fiddling with resolutions and vsync, set it back to how it was previously, re-open it and there are no issues for a while, but then it starts chugging for a brief moment, before returning back to normal. Annoying to say the least, will probably drop it until it's fixed, but I doubt it will be. Dont recall having any of these issues on release.
A similar problem...
The game started at 60+ (although I think there were 144 of them) before the release of the DLC,drawdowns began to 30 fps (and less).
DLC patch messed stuff up, all of a sudden weird 30fps gameplay and terrible stutter.
The last 'Optimization patch' fixed everything - back to normal over here.

Please make sure that your systems are all fully up to date:
Please check for pending updates for your system via the steps below:
- Select the Start button
- Go to Settings > Update & Security > Windows Update
- Select Check for updates
- After installing the update, select Check online for updates from Microsoft Update
Nvidia graphics:- Go to the official NVidia download centre here[www.nvidia.com]
- Using the drop-down menus, fill in your graphics card information and press "Search"
- In the new page, click on Download and then Download again
- Right-click over the file and select Run as administrator
- If you are prompted by Windows User Account Control, click on Yes. The installer will prompt you for an extraction path. Most users should be fine with the default path.
- Once the driver install has completed, the extracted files will automatically be removed from your PC. Choose OK.
- You will next be prompted to accept the NVIDIA software license agreement. Read the contents and then if you agree, click "AGREE AND CONTINUE" to proceed.
- Next, choose Custom (Advanced). Then click NEXT.
- Select Perform clean install and then click on NEXT.
- Once the installation has finished, reboot your PC one more time to complete the installation.
AMD Graphics:- Go to this page[support.amd.com]
- Install ‘AMD Driver Autodetect’ application.
- Select yes on to allow the application to run if the prompt appears.
- Select ‘Download Now’ and follow the steps to install the driver.
- Restart your computer, if necessary after the installation of drivers.
You may also try the following:Setting to High Priority
- Run Blasphemous
- Press the Windows key and type Task Manager and Run as Administrator
- Under the tab "Details", find the .Exe file
- Right-Click and select "Set Priority" and click on "High"
Power Options- Right-Click on your Windows key and click on Power Options
- Click on Additional power settings
- Change your power plan to "High performance"
- Fully shutdown your PC (not just a restart)
- Turn on and retest

Thanks for entering this thread. Since the issue only happens when my 4k monitor is selected as primary, I'm assuming it's a monitor issue. My monitor is U28E590D 28-Inch 4k UHD. It's been overclocked to a maximum of 64 hz.
Could you please confirm the make of the Monitor?
You're able to play with a different monitor that is not 4k?

Yes. My primary monitor is a Samsung U28E590D 28-Inch 4k UHD. My secondary monitor is an LG 25UM57 Ultra Wide. I am able to play without any stuttering on the LG 25UM57 Ultra Wide monitor if I set it to primary.
The Samsung U28E590D 28-Inch 4k UHD creates stuttering no matter what in-game settings I use. Based on what I've seen from some of the other people in this thread saying they have the same problem, at least some of them seem to have 4k monitors.

Can you try changing the resolution of the monitor at all?
This has fixed some other issues including 4k monitors in the past.
Try different resolutions (preferably also try the ones in the game options).
Then re-test the game with these resolutions and see whether there is any difference.

I have changed the resolution both in the game all the way from highest to lowest, and I've also tried selecting a lower resolution through Windows. Neither one stopped the stuttering. I have a 1080 ti, 32GB RAM, and an overclocked i7 6700k at 4.6 Ghz, so I don't think hardware is the issue.
