In the map says it's 100%, but in the save slots shows it's 97.75%

Even if map is 100% you still need to get all base game prayers, all base game mea culpa hearts and rosary beads, and finish the game afterwards, to get 100%. But I think that in ng+ this might be easier to get because of the extra % max limit.

I already got all base game prayers, all base game mea culpa hearts and rosary beads, and yes,i searched and it's like you said, it's only possible to 100 percent the map in the NG+ in the base game the map goes to 98,25~85% at max (if i remenber correctly the percentage).

You also probably need to finish all side quests (Except the gemini one which is optional but you can still get in that cave anyway), get all abilities maxed out, get all caged angels and buy everything from ingame shops. I can't recall if there was anything else. Then finish the game. Thankfully it popped for me while doing the 2nd ending which involved finishing all confessor statues before going to the final boss again.

I'm only missing 8 achievements: Crossing souls, Requiem Aeternam, Bronze Medal, Witness of the miracle, Tirso side quest, to fight Anderas and both endings(on my first run i equiped the true heart of mea culpa and did the third ending).

Yo vi un video, donde explican donde esta todo, pero tambien depende si activaste a las amanecidas, ya que hay que completar todo esa parte del mapa adicional.
