Requiem Aeternam achievement bugged?

Twice. I've completed this game twice killing all the original bosses without using bile flasks but no achievement, and yes I made double sure i wouldn't use the bile flasks by empty them before the boss fight. Is it bugged or am I missing something?

Guess this is the winner. So, basically I'll have to go to the previous room to use bile flasks before boss fights.

Just finished another try and it didn't work. Btw, I'm playing on NG+, I wonder if has anything to do with the achievement.

it should be accumulative or my acheivement is bugged in a different way.

I did a true ending playthrough completely forgetting that esdras would count for that list. I figured that the second phase for crisanta wouldn't count so I used flasks on it, but otherwise used no bile flasks on any of the original bosses.

then I did another playthrough to go for this acheivement beat esdras with no flasks and everything else. then the second I beat crisanta with no flasks the acheivement popped for me. didn't even have to do escribar because I have already done it I assume.

tho I was super paranoid about it. I never used a flask after a boss ever. I would warp to the last prie due and just use it.

also I found these warnings before attempting it

Warning:
Using a Bile Flask in the boss room before or even after the battle can void this trophy. Do not touch your flasks near the bosses rooms.
DO NOT equip the Heart of the Holy Purge, there are multiple reports (myself included) that using this sword heart during a boss battle will void this trophy.

I killed him. Tbh, I really have no idea if you can skip his fight since the achievement involves all the original bosses.
