I have to assume you're talking about the Witness of the Miracle achievement that asks you to do "100%" of the game.
The reason you didn't get the achievement is because you didn't do 100% of the things in the game.
The solution is to do the things you didn't do yet.
The only solution is to check all your unlocked achievments and compare them to a list of all achievements.
I guarrantee you, you didnt achieved everything and the game is not broken or bugged.
The wording you used for the achievement seems wrong, I don't know if this is a language distinction or if you are playing on another platform that has a different set of achievements.
The Penitent One is not a Steam or in-game achievement that I can see.
How many times have you played the game? There are some achievements that are missable and some that are mutually exclusive. Just going through the quests is not enough, you will only get an achievement if you complete the quest the right way. You must play the game at least twice, and make certain to get different outcomes for some achievements.
A few off the top of my head.
Viewing both of the original A and B endings. Completing Eventide can exclude the A ending if you don't follow a very specific series of steps, and the B ending can exclude the A ending as well. There is a very narrow possibility of achieving all three endings in one run through, but you would have to trigger the endings in A, B , C order by following the Eventide quest-line up until the moment you defeat Crisanta, and you would have to abstain from destroying, or at lest completing, the Confessor Statue trials.
Also, because Wounds of Eventide allows you to skip the fight with Esdras, you cannot achieve the Eventide Ending (third ending or C ending) and defeat Esdras, this may make the achievement mutually exclusive with the ending.
Triggering the conversation with Redento after also triggering Cleofas' suicide, before you help Redento reach his destination.
AND, this is mutually exclusive with the Cleofas' suicide, convincing Cleofas' to live.
Saving all of the Kissers, just obtaining the Rosary Knot is irrelevant, you have to actually succeed at the quest.
Triggering Viridianna's death is mutually exclusive with her survival.
You are probably going to have to do some actual research and follow a list of achievements for your game version.
