yes, you'll permanently lose one of your bile flask, but the remaining will have more healing power. I personally suggest to agree to the bargain just once.

once is enough. after that you will obtain some cool rosaries, like being invulnerable when using flask, or doing a lot of damage after drinking from flask, so you want to save them for that moment

I never get the fear of losing flaks, yeah you have less heal potions but they heal you almost full at the end, so instead of wasting time shoving three potions you just waste 1, is way more convenient, also is a good motivation to play smart and try to get hit less.
