What should I do to complete the game on 100%?

Will failure of any quest prevent you from completing the game on 100%?

Collect everything, upgrade everythng, discover everything: relics, hearts, beads, map, etc.

And yes, there are 3 quests, which will prevent you from getting 100% if you fail them: Gemino's quest (if you at least took his quest, technically you can still finish it but lose 0,05 % map), quest from Kissers of wounds (Mediterranean achiev, if even 1 kisser dies - quest is failed. There is empty location right after Albero's Church. If you will find there at least 1 grave - congratulations, quest is failed), and Cleofas quest (plain and simple - he must survive. His quest also has 2 self exclusive outcomes/achievs: Crossing souls (breaks 100%) and Rebirth (required for 100%))

Oh man, thanks a lot for extended explanation. I have already read a lot of articles on the wiki and I was hoping that this small part of the map in the Gemino quest would not affect the completing the game on 100%.

However, will I be able to achieve 100% progress through the New Game Plus?

Yes, NG+ goes way over 100% completion, so you can easily get the 100% achievement even if you fail all quests.

AFAIK losing Gemino quest costs you 0,05% of map, but doesn't prevent you from getting 100% if you brought him required item anyway.

Another question out of the topic: I collected all quicksilver but didn't get the the achievement. With what it can be connected?

PS - I maxed out all my items to Nacimiento, maybe cause of that?

You can use quicksilver to upgrage your bile flasks at Nacimiento (character from DLC). There is no achievs for such actions. I strongly recommend to do so in your main game (like you did), but I strongly NOT recommend to do so in NG+ (or upgrade it only few times). Coz, most likely you'd want to finish it with additional penitences, and full upgrade will cause you more harm than benefit.

If you are on dlc, you could reach more than 100%.
