If I choose to accept her help in the boss fight, will it lead to something bad or affect the ending?

Yes, and if you accept her help everytime, you will eventually get her achievement. There's no downside in accepting it, unless you are going for the "Requiem Aeternam" achievement.

I've already ruined "Requiem Aeternam" by using my own healing pots, so it doesn't matter now. Thanks.

I think you will get the prayer regardless of whether you accept her help or not. But only by accepting her help repeatedly and having her die early you will get the "Last Breath" achivement,

Doesn't matter, she will either give you the relic when she dies or at the end before the final boss.

This achievement looks hard af. And there is one to reach the bronze gate under 3 hours (i hate speedrun achievement). Oh well, no 100% for me on this one.

You can just rush three bosses on an alternative save.
