Can it still be done if you have guilt stuck in a spike pit? I'm not sure if clearing the final boss again will give it back.

I think if you have your fervor as high as your Guilt allows, it should.

Any idea how long you have to wait? Might need to remove my guilt because nothing is happening.

Guilt is probably needed removed then yeah. You don't have to wait more than a few seconds.
