Is anything permanently missable?

I don't think that any item/ability is actually missable, but you can definitely miss some achievements (like the meeting between Redento and Cleofás, Mediterranean Diet, Bronze Medal, not using flasks against bosses, the endings...)

The only things that are not available in one playthrough are certain challenge based achievements; and the "bad" ending becomes unavailable if you complete the quest for the "good" ending.

Even both endings can be gotten in one playthrough by beating the game without doing the Confessor Statue quest, then reloading your save and doing the quest before beating the last boss again.

Nice, wish i thought of that but I wasnt sure if game is playable after killing last boss :) Would have saved me full run to get bad ending :D

Does that mean I'm gonna be able to collect items later which i missed cause i screwed Gemino quest?I can not access the cave which Gemino is supposed to open which contains rosary bead.
Furthermore I can't turn on last healing item to the guy in Albero cause i killed some of the later bosses early.How can i get his rosary knot which is he supposed to give me?

You can access "Gemino cave" by dropping in the near spot with equipped relic that will let you survive all falls (Linen of Golden Thread) thus it is not even mentioned here

Rosary Knot from Tirso is not mentioned here coz as long as you wont kill ALL bosses 'till you turn in all herbs you wont lock it up.
But tbh it should be mentioned, as that achiev for that quest is still missable (you need to turn every herb asap as you get it)
