Quests: Healing Herbs?

I am doing my first ever playthrough of the game right now and I killed some of the bosses before progressing quests. I think I am locked out of the Healing Herbs quest as I have gotten every single one of the herbs but I did not get the extra rosary bead slot. Did I mess something up?

You should get the rosary bead after getting to a certain point (defeating a specific boss).
The idea is to provide a herb before each boss so a patient doesn't die. Even if they die you can still get the bead you'll simply be locked out of the achievement for keeping them all alive.
