Game does not support 4:3
It seems that non 16:9 aspect ratios are not properly supported, at least by the demo. I think the problems with 21:9 have been thoroughly reported, so let me chime in about 4:3, the old standard. This is the aspect ratio of old computer monitors and SD televisions.

Blasphemous reports via menus that it will run at resolutions such as 480x360 and even 320x240, but when attempting to run the game at these resolutions, the game's menus act strangely and when you get in game, the screen is black (although music and sfx still play). I also tried running a more 'modern resolution' in 4:3, 1920x1440, and that gave me the same results: black screen with music and sfx after the opening menus.

The 16:9 resolution of 640x360 works, and that does seem to be the target resolution for this game's artwork and assets. It would be nice to be able to play in 4:3, though, so that I can use all of the vertical space on my 4:3 display instead of having the image letterboxed.

This is in stark contrast to what was reported by the Blasphemous team about their own rendering solution back in 2017 on kickstarter., which really made me think this game was being designed to work with 4:3 and 21:9, both monitor aspect ratios I own and play games on...

It does not, but I did try messing with the registry to get it to run at 4:3 resolutions. The game will happily switch to these resolutions and then go black. Even in windowed mode, this happens.

That's strange. I tried setting the game on 1024x768 on my 4:3 monitor and it ran smoothly.

That...... is VERY strange. Is it letterboxed or just straight up 4:3? Can I get the other details about your setup? Are you running more than one display?

Old Samsung 4:3 monitor, Nvidia GeForce 630 GT video card, Intel Dual Core @1.9 GHZ CPU, 3 GB RAM. I'm playing on my laptop mostly, but I tried it on my PC as well. Like I said, there were no letterboxing, although I'll admit there were small black borders on the side, but the game screen covered 98% of my display. Oh, and I've set the display to "Stretch" in the options menu. And no, I'm using one display.

Ok, why are you "stretching" the resolution? One of the main benefits of CRT's is they can run at any resolution natively, so you don't have to stretch anything in software and give blurry scaling artifacts.

So you want to be playing this game at it's native resolution, which is 640x360 (640x480 if you're letting the game letterbox), or some multiple thereof, like 1280x720 (1280x960 with the letterbox).

Now, if the game actually can crop the sides with these registry hacks and still be playable, giving you a 480x360 native resolution, then you can run your CRT at 480x360@120hz, or 960x720@60hz or 120hz, etc.

yeah CRT are weird. Capcom games were 480x224 pixels or something like that yet 4:3 aspect ratio.
Yeah, old games didn't typically use square pixels, but this game definitely does.

I'm not stretching anything. Read my OP. The devs said that we would be able to run the game at any aspect ratio in 2017. Like I said, I was able to get the game to run nicely at 640x360. I even took pictures, you can see them over here: https://imgur.com/a/UTcav9E

This has nothing to do with that. Actually I tried running it at 4:3 on multiple displays and not just CRTs and got the same result: black screen.

Didn't know that. Wonder if they only meant 16:9 and wider. Because the only 16:9-designed game I know of that also supports 4:3 proper is Sonic Mania, since the camera pans with Sonic as he moves around the level.
