Equip the fire resistance item and just endure it. Don't get hit at the beggining of the fight and you should have no problem taking some hits in the second half.
You want to learn to avoid everything. That fireball spam is unavoidable afaik.
I took the boss down after a few tries but that spastic fireball attack is really cheap on an otherwise decently designed boss fight.

So, for anyone else who comes to this thread wondering why the first real boss is so hard: you're doing them out of order. :D
Unfortunately this is a pitfall you sort of have to have intuition with when it comes to the roguelike genre. I think backtracking out of an area and going to fight another boss usually makes one go “ohhhh this makes sense” and adapt accordingly. Sometimes you can beat harder bosses out of order, and sadly that sort of free path-taking is only available to seasoned players.
And then theres the people who die to tutorial bosses and blame the game...thats a whole other story lol.

The 4 way fireball spam in the second phase seems always fire at double/triple rate making it basically impossible to dodge and the hand doesn't fly a little away to do it to me like seems to in the video. I'm also finding the hit detection/box on the red orbs occasionaly iffy. Sometimes watch my sword pass right through, even after hitting several in a row no trouble.
The attacks shown in the video are piss easy compared to what I'm going up against every try on second phase. Sadly no fire resistance item so trying magic resistance and only have 2 heals
[edit] Eventually forced thru it. Absolutely awful.
The strategy I ended up using in case it helps anyone else (I reccommend don't, come back later) - The first phase got the health bar down close to halfway. Then carried on avoiding everything till get lucky the head & hand are arranged such that could can knock a whole string of red orbs into the head. That gives a good head start on the second phase and just hope it doesn't spam the rapid fire fireballs repeatedly. The lasers are easy enough to avoid so if lucky get enough attacks from those to dodge whilst dealing the pain.
This isn't a boast but how screwed the second phase of that fight was. It took probably a couple hours get through that battle yet I went on to defeat the next 3 [Perpetva, Tres Angustias & Ten Piedad] all first time without even needing to use one health vial.
