If, however, your pet is only Incapacitated (white circle over their head/body), then:
1) Craft Revive Incense (at a Workbench)
2) Equip the Revive Incense
3) Stand next to the Pet, and Face them
4) <L-Click and Hold for a moment (to "charge it"?)>
5) Release <L-Click>; you should see an Incense Cloud if you did it correctly
6) Pet should Revive (with a reduced Health Bar)
7) Heal Pet with Food and/or Potion(s)
Pets will only eat if they are "following" you.
* Pets can not be Revived while underwater (even in Immortal Mode)
* An Incapacitated Pet will Permanently Die if not Revived "soon"
* You can make Pets Immortal in the Game World settings
They can still be incapacitated/revived, but will not permanently die (even if underwater).
If that doesn't work, your game is glitched and probably needs a Bug Report.
I wasted like 5 incense before realizing this.
Just like petting a tame staying greyed out (issues when you are not the host of the world) the same happens when using incense (doesn't work when it should).
Downed pets (especially with immortal pets turned ON) will respawn on your bed / treetop on your next relog which might be an eazier solution in general. A cheaper one for sure (no incense required)
