Unable to get the complete skeletal transformation

I managed to do this once. In my latest play-through, I only managed to get both arms and both legs. However, the head, (skull), and body will not transform. I have attempted to get both head and body to transform completely into a skeleton by saving the legacy character.

Any idea what I could be doing wrong?

I have even put the game in developer mode ... and I get the Oldwane Progression error ... for "Oldwane's price" ... "cannot find piece" fail message at the successful end of any combat encounter.
