Who is V19?
A Charlotte Wiltshire of a different timeline who was also aware of the fake realm's nature.

Where did it start? With Charles' creation, in his world? With the Oracle? With V19? With Scarlett/Yellow-ribbon Charlotte birth?
Chronologically, with Charles. As stated through Vincent's beliefs of the afterlife; once someone dies, their soul cube releases the information stored within it in order to create a new universe out of its contents. So that's how the creation of the Fake Realm came to be.
Technically though, everything all started when Seth/Charles' dad made Lilith/Charles' mom abort Scarlett even though she didn't want to.
Where are we in the end? In a game? In real life? In etherane's mind?
In the literal sense; in a game based off of various aspects of etherane.
Though where exactly Q84 and Charles are at the very end and what they decide to do afterwards with their newfound freedom is up to interpretation.

The last question I have is : who are we? What is supposed to be the protagonist? Why is he named Seth or Lilith? Does he/she even exist?
Now if THAT ain't the million dollar question lmfao but no problem! I'll do my best to answer!
It really depends on who you consider the protagonist?
If you mean like... the puppeteer, then I guess the closest answer I can say is that that we're simply the audience of the story. Charles' only efficient way of communicating/expressing himself to others (since it allowed him to befriend Anri and Vincent, as well as make his mother happy to an extent) is through art and storytelling. So that's why the nature of the world he created turned out to be a place that births countless different stories.
"But slothmama, what about the Seth cameos in the first game?!" Weeell... the puppeteer has always been strictly limited to their puppet's perception of the world, and the Umbrella Man DID say that it never actually happened. But it really depends on you on whether or not this Seth[hellocharlotte.wikia.com] ever actually existed or not.
Scarlett just generally names the player Seth/Lilith because that's just what she wanted to name us. And also maybe because we come off as a parental figure to her considering how we're basically watching over her so that's why she names us after one of her parents? idk man that part just makes me Feel things and cry
As Etherane pulled somewhere in the game the"DDLC trick" with Q84 adressing to what she calls the player (and even the viewer), I wasn't quite sure if Seth/Lilith even existed. Their existence really depends on the layer where the protagonists interract.
We can't denie we had a part in this story. We watched their story. We felt things. We agreed with them, or not.
And that's all that mattered. Etherane is wrong. Many people suffered, there was things to be sad about.
As long as there would be people to feel sad or happy, as long as there would be puppeteers enjoying these stories and remembering them, nothing will be over, and Charles will be better. We will write other stories. Etherane will write other stories.
Because some stories aren't meant to be ended.
(Though I'm like, 100% sure that Etherane was just lying about how there was nothing to be sad about. Like how one would say "Yeah... I'm Fine." even though they're going through a tough time because they don't want to trouble people. Like how Charles always felt like a burden half of the time towards others.)
