There's a simple explanation for how the Hearthians survived the ghost matter; Water is ghost matter's weakness - notice that you'll never find ghost matter underwater, even in areas where it is otherwise everywhere. For instance there's a little workshop in the Stranger's wetlands near the beginning where you can find a lantern artifact. It is surrounded by ghost matter, but there is none in the stream that runs underneath it.
Now at the time the interloper exploded and scattered the ghost matter killing everything, where were the Hearthians? According to the Nomai, they were aquatic at that point in their evolution! Hence why the ghost matter didn't touch them at all!
Ghost matter decays over time. Heck, they tell you that back in the village at Timber Hearth at the ghost matter patch there, that it used to be bigger. It has been a long long time since the Interloper exploded, so most of the ghost matter is gone by now.
They never met the Nomai because they never wanted to find or be found by anyone, they just wanted to return to their home moon in the only way they could. At most they had one of their lifespan's left before they couldn't leave the simulation due to their physical bodies dying. We also don't know how long it took for the Prisoner's leaked signal to reach the Nomai, the Strangers' bodies may have all already been dead by the time the Nomai warped in to Dark Bramble.
As for water nullifying the effects of Ghost Matter, this was true in the base game as well. On Giant’s Deep there is a small tunnel near Gabbro with a small amount of Ghost Matter. When his island lands in the water after being tossed, the water will briefly fill the tunnel and you can walk through quickly without being harmed.
