They're easy enough to avoid when they're first encountered swimming through the narrow passageways filled with weeds to hide in, but in the giant underwater cavern (Kawaq Yaku) where there are like 10 schools swimming around, they're hard as hell.
It's a BS enemy too because piranhas don't just attack swimmers like this. Lara would have to be clearly injured, struggling to swim, and bleeding profusely to actually draw their attention IRL. Instead in that cavern, if you're anywhere within 50 feet of them and they spot you, you're done. There's no fighting them off or escaping. You get to watch her get eaten alive and then restart from you last save. There are effectively no weeds to hide in and swimming up or down to try to pass over/below doesn't seem to work. I've been trying to get the treasure chest in there for 20 minutes and the slightest movement away from the sunken temple with the air hole results in death.
Far be it from me to to complain about something unrealistic in a fantasy game about supernatural relics and burial chambers filled with complex traps and puzzles, but the piranhas -- attacking a healthy swimmer with no gaping wounds pouring blood into the water -- is f'ing nonsense.
That said I am trying to reach the treasure chest from the exit (coming from the Airfield Dirt Road camp), so maybe going through there the original way will make it easier.
