I suspect most just wouldn't understand her? It's not like we're in a tourist spot where everyone speaks english.
However do keep in mind there are plenty of players that aren't from North nor South America.
Isn't "♥♥♥♥♥♥/Gringa" used to refer to American citizens though, while Lara is English?
At least they had the voice-over immersion as an option. Too many games these days have voice-actors doing blatantly fake and over-exaggerated anglicised foreign accents while trying to sound like a native from a country represented in-game.
Got a good laugh out of Graham McTavish trying to pronounce "Dyfed" - a Welsh word - in Hunted: The Demon's Forge. :D
Right, in Japan. But we're talking about an unspoiled hidden city in Peru where people are still wearing loin cloths and ♥♥♥♥... not Japan.
Which probably requires a complete change of voice actresses as Camilla Luddington probably doesn't speak Spanish (and then it would need to be Mexican and Peruvian dialects, not regular Spanish.)
But then, half the NPCs, such as Uchu, still speak English (with fake/exaggerated stereotypical Spanish accents) to Lara even with Voice-Over Immersion active.
It ruins the immersion more rather than enchanche it in middle of hecking nowhere in Peru, Lara I can get, but not from a dude who was just whittling flint to stick on his arrowhead and that includes the hidden city denizens. Who can understand english well enough, for *some* reason.

Now Lara speaks Spanish to them and they answer with their local language, with English subtitles for everything.
(Actually all audio is in Spanish now, but since Spanish language pack also includes English audio, I can change between Spanish/English audio ingame if I want to hear English for some parts of game.)
