It's not a PC bug so much as a gamepad vs keyboard bug. Not the first nor the last time that'll happen either. You're generally better off not using your mouse while climbing, as that will usually screw with the camera and thus angles will be wrong for the jumps. It's stupid, I know, but alas, we are all victims to the console era.
There's also a possibility that you're not actually at the top of the climbable area. This is what I refer to as the "climbing axe glitch". It can happen in the middle of a climbable area sometimes that Lara simply stops climbing as if she's reached the top, and looks like she wants to jump upwards. Sometimes she'll even grab a wall outside the climbable area, and as you start moving, simply slip and fall. Very glitchy. Try dropping into a repel and climbing back up - that usually fixes that glitch.

i played all the modern era tomb rider (the last trilogy) and i never realized that there was a difference between jumping up or diagonal
i was stuck at that damn wall jump for 2 hours

I have watched 3 videos of the tomb on youtube and all the players make the ledge with ease, they do like a double grab, which I am doing as well, and they make the ledge, but I fall way short each time. I do not want to skip this tomb, as I want to complete the game, but I am at a loss about what to do?
Anyone else having this issue and does anyone have a solution? Perhaps I am missing a skill?

I did it in the end by turning the screen resolution down to 800x600 in the options, made it easy then :)
If anyone else is having this issue, I would recommend trying the resolution method first.

BTW, there's another bug - if you revert the screen resolution back to 1920x1080 - the interface texts are distorted. What you need to do is to change resolution to some other (i.e. 1620x1050) and back to your original. it worked for me!
If you use the jump, she will not jump "high" enough to make it to the ledge.
