A good mix for harder difficultys is


Mantle of Six Sky
Boots of the Empress Jaguar

Or the complete outfit for both if you're more into fashion. They offer good perks.
There's not really that one pro outfit, they all have good perks depending on your playstyle.

On my first easy run i usually ran around in commando. for the looks. :-)



Standard issue Trinity firearms are all very good for what you have to do, but ...

Pistol changed to nep.14 for CQB - massive dmg point blank, but low ammo pool.
I switched out for the fal on my rifle slot - personal prefernce, you loose out on fire rate.
Standard shotgun is great, but i did switch for the whws-16 later on. more fun.

for the bow i played majority with upgraded serpentwood, after i used viper bite on NG+
