Black on all sides can be resolved in your graphics driver settings. Change your screen resolution to 1280x960 and make sure the scaling mode is set to "Maintain Aspect Ratio" or something like that, then go back to 1920x1080. Relaunch Touhou 16 and it should be properly fullscreen.
Black on just the left & right sides is normal. A 4:3 game should not be stretched to fill a 16:9 monitor.

Seriously, though. If the highest res available was 1280x720 instead of 1280x960, that'd allow the game scale properly for the majority of 16:9 monitors in fullscreen mode. Why this isn't the case already is beyond my capability to comprehend.

And yes, Zun is drunk when making this. Don't think about it too much.
Try making a custom 1280x960 resolution in your graphics drivers.
How is going from 1280x960 to 1280x720, increasing the resolution?
♥♥♥♥♥♥♥ duh, I think I misread your post. Still, an indie game dev should know why 1280x720 is a better option, instead of putting effort into something that'll scale pretty bad on the vast majority of modern widescreen displays.
Uh... in actuality it would actually decrease the resolution. The fact of matter is, 1280 x 960 is an uncommon resolution in monitors. I'd prefer 1280 x 720 of course, but since ZUN is drunk when making this, best not to think too much about his decision.
What I meant to say was increased horizontal ratio in a verticle shmup.
