For the slayer you need to have seen a pop up on the home screen showing a fairy in a bottle.

Literally made a guide with those endings and all others from the DLC ;)
If you wish to be a collaborator on the guide so you can add more info there, such as what the choice is (since I wrote what the stats were but not what the choice was), you're welcome to add me as a Steam friend so I can add you to the guide and you can edit it, or if you want to make a guide yourself, you're welcome to, too, but making such a thread is not really useful, as it will eventually be pushed to the 2nd page and then further and further in the abyss unless you bump it regularly... and people won't really look for it if they need help with an ending.
