This 4.0 world builder also has settings for continents/islands and land to water ratios - these can limit land masees and the distribution of cities also has an impact.
The Simple Map WB or The 3.11 WB are better at producing maps with islands with a few cities on them if that is what you are looking for.
Also,if you are not finding a second city until turn 95, the city settings values are most likely set up for cities to be quite scare. Again, the 3.11 WB has less options but more generalizes distribution which it sounds like oyu are looking for.
When using that World Builder, you really have to consider what you are wanting to achieve, and then experiment with it. The most important settings in terms of world building are Land To Water and continents to islands.
