Can some one tell me how to play it? Not in the dlc?

On Stage 5, I understand both fuses need to be found and placed and then you press Q after he states that "now maybe the door can be opened". The problem I'm having is the gated door up the stairs on the left hand side is not opened after I have pressed Q to exit the fuse box. This is the same room where I had to reach through the bars to pickup the second fuse. I took two screenshots to show you but I cannot post them here. Seems like a bug. So I went to the Option menu. It just displays vsync, resolution, and fullscreen. Where are the sound settings? Control settings? (Yes, I know you can get invert-Y, non-listed resolution by changing main.cfg). But how dya set the mouse speed/accellaration/etc? Is there a secret button somewhere in the option menu that I miss?...
