You have not saved your game from level 4 to level 41? Do you realize this is a Bethesda game? You MUST do a HARD save VERY OFTEN. I recommend every level, or even more.

If my calculations are correct Bethseda auto saves are more than appropriate to run my game on; but man am I bad at math.
lost serana there myself just now and used a pretty easy fix.
save the game -> load an older save where serana is still there -> open the console and left click serana to get her ID to show in the console bar -> load your latest save -> open console again and type "moveto player" without quotes
according to wiki placeatme changes some stuff which might be your problem..
moveto player Teleports the targeted NPC to the Dragonborn. (Works similar to player.placeatme <BaseID> <#> but does not create a new reference of the base object.)
(Also, I just got done playing through the Soul Cairn and I'll probably be looking for a mod that does map markers for Jiub's Opus if I go back, because apparently those don't show up even when you activate the quest.)
Necro just to show better fix I found.
Placeatme works, but creates another instance of Serana. No bueno.
Use moveto.

(xx for dawnguard load order, mine is 02. prid selects that ref.)

I couldn't fast travel or anything, so I just started looking. Found the "Reaper'a Lair",went inside to see what it was, and she was standing there. As soon as I started walking around, she went back outside and was waiting for me. Somehow she entered, and then got stuck, I guess.
